SDMNY Sign Up Session

Attend a Zoom Sign Up Session to enroll in the SDMNY Facilitation process.  Sign Up Sessions provide individuals [prospective Decision-Makers (DMs)] with information on the SDMNY facilitation process, explaining roles and responsibilities of all participants in the process and answering questions on next steps to achieving an SDMNY Agreement.

This session is intended for the individuals (potential Decision-Makers) who will participate in the SDM facilitation process to create their agreements. Attendance by the individual (DM) who will participate in the SDMNY process is required.
